GLP-1 Arrived Frozen? Winter Cold-Chain Rules and What to Ask Your Platform
If your GLP-1 pen or vial arrived frozen, or you can see ice in it, do not use it, even after it thaws. The labels for Wegovy, Zepbound, Mounjaro, Ozempic and Saxenda all say not to use the product if it has been frozen. Set it aside, document what arrived, and contact the pharmacy that shipped it for a replacement before your next dose is due.
Key takeaways
- Every injectable GLP-1 label covered here says do not freeze, and do not use if frozen. Thawing does not reverse the problem.
- Refrigerated storage for these products is 36°F to 46°F. Cold-weather problems come from going below that range, not only above it.
- Cold packs plus a freezing porch or unheated truck can push a package below the safe range during winter transit.
- Take photos, keep the packaging, and contact the dispensing pharmacy and your platform the same day.
- USPS recommends shipping by December 17 to 19, 2026, depending on service, for delivery by Christmas. Plan refills around holiday closures.
What the labels say about freezing
The freezing rule is short and the same across brands. The Ozempic label (revised May 2026) puts it this way:
Do not freeze OZEMPIC® and do not use OZEMPIC® if it has been frozen.
Lilly's tirzepatide labels, the Zepbound prescribing information (revised August 2026) and the Mounjaro label (revised August 2026), carry the same do-not-freeze, do-not-use-if-frozen instruction. Novo Nordisk's Wegovy pen instructions say to throw the pen away if Wegovy has been frozen. The Saxenda label (revised February 2026) adds a detail many people miss: keep the pen away from the refrigerator's cooling element, since a home fridge can freeze a pen too.
The labels do not offer a "thaw and inspect" option. A pen that looks normal after warming up is still a pen that was frozen.
Storage ranges, product by product
Each product has a refrigerated range plus a limited room-temperature allowance. Knowing these helps you judge whether a cold package is a problem, and what to do if your fridge runs too cold.
| Product | Refrigerated | Out of the fridge | If frozen |
|---|---|---|---|
| Wegovy pen | 36°F to 46°F | 46°F to 86°F in the original carton, up to 28 days | Throw away |
| Zepbound / Mounjaro single-dose pen or vial | 36°F to 46°F | Up to 86°F, up to a total of 21 days | Do not use |
| Zepbound / Mounjaro multi-dose vial or KwikPen | 36°F to 46°F before first use | Follow the after-first-use limit in the label or carton; ask the pharmacist if unsure | Do not use |
| Ozempic pen | 36°F to 46°F before first use | After first use, 56 days at 59°F to 86°F or refrigerated | Do not use |
| Saxenda pen | 36°F to 46°F before first use | After first use, 30 days at 59°F to 86°F or refrigerated | Do not use |
| Foundayo tablets | Not refrigerated | Store at room temperature, 68°F to 77°F | Ask the pharmacist about exposure below 59°F |

Two notes on the table. First, the room-temperature allowances are about warmth, not cold; none of them permits freezing. Second, the Wegovy pill and Foundayo are tablets with their own storage rules. The Foundayo label sets a room-temperature range, so a tablet bottle left in a freezing mailbox is outside that range. Ask the pharmacist rather than guessing.
How winter shipping freezes a package
Cold-chain packaging is designed to keep refrigerated medicine cool on the way to you, usually with insulation and cold packs. That works against summer heat. In winter, the same packaging can work against you:
- Cold packs plus cold air. Packs meant to offset heat have little to offset when the truck, sorting facility or porch is already below freezing.
- Long dwell times. A package that sits outside for hours after delivery, or overnight in an unheated vehicle or facility during a delay, keeps losing heat.
- Weather and holiday backlogs. Storms and peak-season volume add days in transit that the packaging may not have been designed for.
- Mailboxes and lockers. Metal boxes and outdoor parcel lockers do not protect against freezing.
You cannot control the carrier, but you can control the last stretch: delivery address, timing, and how fast the package gets inside.

How to tell if it froze
You do not need a lab to make this call. Signs a product froze or may have frozen:
- You can see ice crystals or slush in the pen window or vial.
- The liquid looks solid, cloudy or has particles while cold.
- The cold packs in the box are frozen solid and the medicine was touching them, in a box that sat outside in freezing weather.
- A temperature indicator in the package (if the pharmacy includes one) shows an out-of-range reading.
If you are unsure, treat it as frozen and ask the pharmacy. The cost of a replacement is lower than the risk of injecting a product that may not work as intended.
What to do, step by step
- Do not inject it, and do not throw it away yet. Put it in the fridge, labeled "do not use," until the pharmacy tells you whether they need it back.
- Photograph everything: the box as delivered, the cold packs, the pen or vial, lot and expiration on the carton, and any temperature indicator.
- Save the tracking history showing delivery time and any delays.
- Contact the dispensing pharmacy first. The pharmacy name is on the prescription label, and it may not be the same company as your telehealth platform.
- Message your platform too, so your prescriber knows a dose may be late.
- Ask about timing: "When will a replacement ship, and will it arrive before my next dose day?"
- Get the outcome in writing: replacement, refund or credit.
Questions to ask your platform before winter
| Question | Why it matters | What a good answer looks like |
|---|---|---|
| Do you change packaging for freezing weather? | Summer packaging can over-cool in winter | A specific description, not "we follow cold-chain best practices" |
| Can I require a signature or hold at a location? | Cuts time on a cold porch | Yes, with steps to set it up |
| Can I ship to my workplace? | Someone is there to bring it inside | Yes, or a clear reason why not |
| What is your replacement policy for frozen product? | You need a dose in time | Written policy with a timeframe |
| Will you avoid shipping right before weekends and holidays? | Packages can sit in transit | Ship dates chosen to avoid long dwell |
These are process questions; answers vary by pharmacy, so verify directly.
Holiday-season timing
Winter weather and holiday volume overlap. For 2026, USPS recommends sending by Thursday, December 17 (Ground Advantage), Friday, December 18 (Priority Mail) and Saturday, December 19 (Priority Mail Express) for delivery by December 25 in the contiguous US, per its holiday shipping dates page. Pharmacies may use other carriers, but the same squeeze applies.
Federal holidays this season fall on November 11 and November 26, 2026, December 25, 2026, and January 1, 2027, according to OPM. If your refill would normally ship that week, ask your platform whether it can ship earlier, within the refill rules your prescriber and plan allow.
If the replacement is late
A frozen shipment can turn into a missed dose. The labels' missed-dose windows set the outer limits. The Wegovy label says to take a missed injection as soon as possible if the next dose is more than 2 days away, otherwise skip it, and to restart dose escalation at a lower dose if 2 or more consecutive doses are missed. The Zepbound label allows a missed dose within 4 days (96 hours); after that, skip it and resume on the regular day. Tell your prescriber the date of your last dose so they can decide whether you should restart at the same dose.
A hypothetical scenario
This is an illustration, not a real patient.
Your box arrives at 9 a.m. on a 15°F morning and you find it at 6 p.m. The cold packs are rock-hard and the pen window shows slush. You photograph everything, put the pen in the fridge marked "do not use," and message the pharmacy and platform with the photos and tracking. Your injection day is in three days, so you ask whether a replacement can be overnighted, and you ask the platform to require a signature on future winter deliveries.
Where this guide stops
This page covers label storage rules and shipping logistics. It cannot tell you whether a specific product is still usable, which is why the default is to not use it and ask the pharmacy. It also cannot set your restart dose after a gap; that is your prescriber's call.

Frequently asked questions
Can I use my Wegovy or Zepbound pen if it froze and then thawed?
No. The Wegovy, Zepbound, Mounjaro, Ozempic and Saxenda labels or manufacturer instructions say not to use the product if it has been frozen, and thawing does not change that. Contact the dispensing pharmacy for a replacement.
What temperature should GLP-1 pens be stored at?
Refrigerated storage for Wegovy, Zepbound, Mounjaro, Ozempic and Saxenda is 36°F to 46°F. Each has a limited room-temperature allowance, such as up to 28 days for Wegovy pens and up to 21 days for Zepbound single-dose pens and vials.
What should I do if my GLP-1 shipment arrived frozen?
Don't inject it. Photograph the box, cold packs, product and lot number, save the tracking history, and contact the dispensing pharmacy and your telehealth platform the same day to arrange a replacement before your next dose.
When should I order a refill around the holidays?
USPS recommends sending by December 17 to 19, 2026, depending on service, for delivery by Christmas in the contiguous US. Ask your platform whether a refill due that week can ship earlier within your prescription's refill rules.
