The era of injection-only GLP-1 treatment is over. Two oral GLP-1 medications are now FDA-approved for weight management: Foundayo (orforglipron), approved April 1, 2026, and oral Wegovy (semaglutide tablets), approved May 7, 2026. Both are available through telehealth. Here's what patients considering oral alternatives need to know.

Foundayo (Orforglipron): The Game Changer

Foundayo is the first small-molecule, non-peptide oral GLP-1 receptor agonist. That technical distinction has a practical consequence that matters enormously: you can take it any time of day, with or without food, with no water restrictions. No other oral GLP-1 works this way.

Unlike semaglutide and tirzepatide (which are peptide molecules that degrade in stomach acid), orforglipron is structurally simpler and absorbs reliably regardless of food or drink. This eliminates the 30-minute fasting window required by oral semaglutide โ€” a restriction that, in practice, caused many patients to abandon the oral form for injections.

Dosing: Starting dose is 0.8 mg once daily, increasing to 2.5 mg after 30 days, then 5.5 mg after another 30 days. Further increases to 9 mg, 14.5 mg, or 17.2 mg at 30-day intervals based on response. The titration schedule is slower than injectable semaglutide but faster than tirzepatide.

Pricing: Eli Lilly launched Foundayo at $149/month for the starting dose through LillyDirect. Subsequent doses range from $199 to $299/month. For insured patients with coverage, copays may be as low as $25/month. Medicare Part D beneficiaries may access Foundayo for approximately $50/month under the CMS BALANCE model starting July 2026.

Availability: Available through LillyDirect, Amazon Pharmacy, retail pharmacies, and telehealth providers. Several GLP-1 telehealth platforms have already added Foundayo to their menus.

Oral Wegovy (Semaglutide Tablets)

The FDA approved oral Wegovy on May 7, 2026 โ€” a daily tablet form of the same semaglutide molecule used in injectable Wegovy. This is a peptide-based oral formulation, which means it requires specific administration: take on an empty stomach with no more than 4 ounces of water, then wait at least 30 minutes before eating, drinking, or taking other medications.

This fasting requirement is the key practical difference from Foundayo. Some patients find it manageable (take the pill immediately upon waking, then go about your morning routine for 30 minutes before breakfast). Others find it disruptive enough to affect adherence โ€” especially those with morning medication regimens or work schedules that don't accommodate a fasting window.

Efficacy: Clinical data for oral Wegovy shows weight loss comparable to the injectable form. The same mechanism of action and same molecule means you're getting the same pharmacological effect โ€” the difference is the delivery route, not the drug.

Oral vs. Injectable: The Trade-Offs

Convenience: Oral medications win for patients who dislike needles, travel frequently (no cold-chain storage or syringes to carry), or simply prefer taking a pill. Injectable medications have the advantage of once-weekly dosing โ€” one injection per week vs. a pill every day.

Absorption consistency: Injectable GLP-1 delivers the full dose directly into subcutaneous tissue with near-100% bioavailability. Oral peptides (like oral Wegovy) have lower and more variable bioavailability โ€” the dose in the pill is much higher than the absorbed dose, and absorption varies with food intake and individual GI conditions. Foundayo's small-molecule structure gives it more consistent oral absorption than peptide-based oral formulations.

Side effects: Gastrointestinal side effects (nausea, diarrhea, vomiting) are common with both oral and injectable GLP-1s. Oral formulations may cause more local GI irritation initially. The overall side effect profile is similar across delivery routes.

Cost: Foundayo at $149โ€“299/month is in the same range as compounded injectable semaglutide ($130โ€“200/month). Oral Wegovy is priced comparably to injectable Wegovy ($1,300+/month retail, potentially lower with insurance or savings programs). Compounded oral GLP-1 is not currently available โ€” Foundayo and oral Wegovy are brand-name only.

Getting Oral GLP-1 Through Telehealth

Both Foundayo and oral Wegovy are available through telehealth platforms. The prescribing process is identical to injectable GLP-1: intake, consultation, prescription, pharmacy fulfillment. The difference is that oral medications ship without cold-chain requirements โ€” no ice packs, no insulated packaging, no concerns about the package sitting on your porch in summer heat. This simplifies logistics significantly.

For patients who prefer injectable compounded semaglutide at lower cost, Embody remains the top pick at $149/month with full clinical support. The choice between oral brand-name and injectable compounded comes down to personal preference and budget.

Which oral option? If convenience is your priority (no fasting, no food restrictions), Foundayo is the better oral choice. If you want the most-studied molecule with the longest safety track record, oral Wegovy gives you semaglutide in pill form โ€” but with fasting restrictions. If budget is the primary concern, compounded injectable semaglutide at $130โ€“200/month remains the most affordable GLP-1 pathway.
